Day 13 (6/10/09) 24 more days left and 15 more days to Osaka!!!!! Ok today is my second last day in Seirei Mikatahara Byoin Ward C4...so this is what i did today...in the morning...saw how Chida san prepare the stuff for injection and set drip...its so so different...no usage of kidney dish but a plastic bag...and no need for sterility...wow...it is super super different...then for intramuscular injection, they have to rub the injected region...and one patient requires a drip but she has no plug, so chida san just set a plug straight away...nurses here can really do everything...but she set the plug really fast and good and neat...goodness, even doctors in Sg cant even do it la...people like chida san can do it fast and neat also put protective sheet...those idiot doctors who know they are going to spill blood everywhere, pls be considerate and put protective sheets ma...make nurses do more work nia!!!!!
haha and their sharp box(the box we use to throw needle in Singapore-ours is yellow) is red...the best thing is, no only do they throw needles, they also throw used ampole( glass or plastic) sometimes syringes also goes in. Their medicine order and patient records are in the computer so all the drips and injection solution are prepared by their material dept and send up to the ward...meaning in a trolley, there is the whole ward supply of drips and injections required for the day...convienent lor!!!so they just have to remove and double check, prepare and serve...and they sign off using chop...or stamp same in sg but just that theirs is in red and is like those chinese way of stamp...one round circle with their surname in knaji(chi characters) inside the circle. haha and the plug in Sg only can keep for 3 days...sometimes we only change on the 5th or 6th day...in Japan, the plug can be kept max 1wk...and if condition is good can be kept till 2wks...like WTH!!!!! haha...
to make work easier for the stuff, they actually have signs that read as 'today is my bathing day' hung on the head of the bed of the patient...haha...and patients can wash their clothes in the hospital if they like...100yen for 1hr...then after that they can spin dry their clothes for 100yen/ hour...so cool...
for patients whom they need to measure urine and ask the patient to press callbell when she needs to go to the toilet, but yet the patient refuse to press the callbell, the nurses put a mat on the floor so when the patient stands up on the mat, the nurses are aware because its a call mat!!!! haha...like totally cool!!!
drugs are also different...we only have one locked cabinet for controlled drugs, they have 3 locked cabinet...1 for poison, 1 for narcortics and one for anti-hypnotics...and the record book is out in the open...only anti-hypnotics, narcortics and another type of drugs needs to be checked daily...the other type of drugs are emergency drugs for high fever, high blood pressure, heart problems, cold, etc...all must be ordered by dr before they can be served.
like i say, the nurses here really can do anything...chida san even took blood culture for one of the patients!!! culture to see any bacteria in the blood...usually in Sg is done by doctors, but nurses here really have to do it...they dont even have cleaners here...hence, all the rubbish that they throw, they have to clear it themselves...just that their bins are not as dirty as ours...theirs is really clean...
taking blood sugar level has no difference, just that they uses a injection pen while we use disposal needles...and their insulin is via insulin pen...no insulin needle...
so yup, after lunch, went to dialysis room to see see...they have so many beds for the patients! and patients lie on bed while they are having dialysis and they even have a small tv screen to watch TV...each session takes about 4 hrs and 3x a wk they have to go for dialysis...so sad la...and as the dialysis fluid is very expensive, the hospital decided to make their own fluid...is hard work...machine A will dissolve powder A, machine B will dissolve powder B and machine C will clean the water...then all 3 solutions will be mixed together to be dialysis fluid...
after that, i joined the ventilator class which was taught by the staff in the ward to some new staff...was quite alright...chida san translate to me...and i did learnt abit in school so it was not too bad...at 2.30pm, we left the room to see the emergency trolley...they have a defibrillator and according to chida san, is monophasic(one way)...they their e-trolley is never locked and check only once a day and after use...the equipments for endo-tracheal tube insertion are on a tray but it looks kinda messey coz everything is stacked together...then their ambu-bag is already set up and place in a plastic bag...is faster but don't look very clean though....hehe...
after that, she told me about the plans for tomorrow...i will take charge of 2 patients...wah, she freaked me out la! i thought i have to write report and all...i forgt it has to be in japanese...but what she meant is that these 2 patients will be my primary patients and i will be with them the whole day to see how patients live their lives in the hospital...writing report and all, my preceptor will do it...phew! and so happen one of the patient she arranged for me has HIV...and tell me if i dun wan, they can change the patient for me...i thought it was ok...of course, i will be scared at first but being a nurse, you cant choose your patients and we have to be non- judgemental...and i thought it will be a good experience to nurse a HIV patient...with enough protection, it should be no problem!!!!! I can do it! Nothing is impossible!!!!!!
Guess im going to cook dinner agn!!! haha...planning a party this friday as the OT girls are going to go back singapore on monday...and its friday night!!!! time to party!!!!! calling jap gals over too! haha...and at the same time, celebrate shu ling's birthday! haha....looking forward!!!!! WEEEEEEEEEEEEEEE........................
