Day 9 (2/10/09) ok, so today had the first proper attachment...follow preceptor...and very good, my first preceptor, english is her weak point...so ya, i gt help from other people who could speak abit english. the rest of the time, we use actions lor...den some basic jap...haha...but the electronic dict quite ok only leh...haiz....
so today did diaper change, andthe way they do it here is so different. They will use cotton wool, put a little bit of medicated soap and water, rub them together get a bit of bubble then use the wool to clean patients pvt region. then a special bottle (one that looks like our typical sauce dispenser but with many openings and the bottle is long) so they will rinse the patient pvt region with the water. then dap dry with a yellow small towel. yellow towel are for pvt region only.then white towel is for body. So clean right, in sg, one towel for everything. after diaper change, must clear trolley la...haha but they have so many things to clear and you have to sort the type of rubbish and all...lol
followed by that, I observed the procedure for insertion of CVP (a catheter inserted into your neck vessel) it was good. the doctor was really skillful. He could insert it really fast of course with the help of an ultrasound also la...but his hands were firm!!!! really, never shiver at all la!!!!
after that, we serve diet. their diet trolley like ours have 2 cupboards where the food are stored. but in each individual cupboard, it is further divided into 2, one side hot, one side cold. so cool la, then the trays just slot in to gaps.and their trolley, once you touch the sensor, you can drag it. you dont touch the sensor, you dun get to pull the trolley. We fed one patient who have swallowing problem. So instructions given was that his head of the bed have to be 30 degrees. my preceptor really had to get a ruler to measure...and so we fed him a huge amt of thicken ensure...it stinks really!!!! I hate the smell of ensure...lucky in Japan they only have vanilla flavour...im sure i ll puke if the flav that I was feeding is strawberry flav. so because he have problem swallowing, we have to use a spoon like teaspoon but with a longer handle to feed him. and we have to make sure he swallows before we can feed him again...if not mouth overload...so we took a long long time to finally feed more than half...den at 1pm another staff took over to feed...coz we have to go for lunch break.
oh ya, before feeding patient, i observed renal biopsy (taking some tissue specimen from the kidney) in a small treatment room, inside there, there was a ultrasound machine, a microscope, a long table, a small bed, and instrument trolley. with that, there are 4 doctors and 1 technician or is it 5 doctors. dont know, all of them have coat. then one staff nurse to assist, my preceptor and me...hehe...so ya, one doctor hold the ultrasound thing, the other insert the needle to kiap the tissue...when the tissue is out, it was passed to the technician. then they did for four time la...wow...
oh also, today did oral toilet also. meaning to help patients who can't brush their teeth. both countries use the same pink sponge on holder thing...and we both have a cup of water for cleaning the sponge...but whats special in japan is that, they have another cup of warm water where they put in toothpaste powder, and is green tea flav. so u have to soak the sponge and squeeze the water out and clean patients mouth. then later, wash off with the plain water. ya...oh and their gloves are powder free...they only got M and S size la...M can wear but my hands have too much meat...hehe...but was not strechy at all...haha...
haha ya lunch, i ordered bento with the nurses hehe...so yup its veggie and half an egg bento with spag and rice...alrite la...but the box is kinda nice...but you have to return to vender...haha...so aft lunch was 2pm and dr and nurses are suppose to discuss the patient's condition and how to treat them...kinda boring for me cause i cant really understand certain things.or rather everything.hehe...
then 2.30pm i had to choose within bone marrow aspiration and DM class...so I choose DM class coz its delivered by nurses and on every friday only. next friday i wont be here liao...so i opt for that lor...but my partner thinks i shld have went for bone marrow aspiration. bone marrow i nv really see before though but CSF fluid removal should look abit like bone marrow also hor...duno...but whats done is done....no regrets and im having backache la....argh!!!!!
haha and it proves some thing, people with masks on look better than with mask off...haha...3 cases prove it right!!!!! hehe.....ya wkends agn!!!! yay!!!! going to have fun!!!!! weeeeeeeeeeeeee...............................hehe :D
